Your options for 2025 look similar, while also adding a new 3rd plan- PPO Preferred. All three plans include a new benefit - titled Garner. Garner is a free benefit that helps you find the highest quality doctors while saving you up to $9,000 in reimbursement costs for visiting Garner recommended providers.

Garner: Medical concierge that helps you find the highest quality doctors while lowering medical bills. When you use Garner to find a doctor, Garner will cover $1,500 to $9,000 of your out-of-pocket cost from that care.
International Rx Program: You can receive prescription medications at $0 copay by enrolling to receive these medications through international mail order.
Teladoc Health : Virtual Care for non-emergency health needs like prescription refills, coughs, colds, UTIs, sinus allergies and much more. Also includes dermatology and mental health.
HSA Plan members may contribute up to $3,600 per year for individuals and $7,200 for family to their HSA account through Optum Bank. Employees age 55 and older may contribute an additional $1,000 per year. When switching from an HSA Plan to a PPO Plan, an employee does not lose any of their HSA funds or the ability to spend these funds; however, they can no longer make contributions to their HSA account.
The medical plans are administered by Personify. Your Personify team can assist with prescription information, services requiring pre-certification, claims and billing questions, questions about upcoming procedures, and finding an in-network provider. The member portal allows you to view your virtual ID card, your medical claims, and important notifications from Personify.
All employees are required to provide dependent documentation for dependent(s) you’re adding to the medical, dental, or vision plan(s). You will receive an email from the Benefits Service Center following Open Enrollment requesting the verification documents. Be sure to provide the dependent documentation by the deadline indicated in order for your dependent(s) to have coverage for 2025.
Class 1 – All employees in this class are assumed to be Full-time and will not be measured.
Class 2 – All employees in this class are assumed to be Full-time and will not be measured.
Class 3A and 3B - (30+ hours per week) – All employees in this class are assumed to be Full-time and will not be measured.
Class 4 - (20-29.9 hours per week) - Part-time benefits will be offered after an initial waiting period of the first of the month following 30 days. Employees will be subject to ongoing ACA measurement every 12 months. EE benefit classes are subject to adjustment after every measurement period.
Class 5 - (below 20 hours per week): Employees will be subject to ongoing ACA measurement every 12 months. After the ongoing measurement period, employees could test into Class 4. Employee benefit classes are subject to adjustment after every measurement period.
